There might have been previously fraudulent transaction on your account and Regions flag it to make sure they check IDs. Fraud is rampant these days. I can't tell you how bad it's been.

My DL is rejected a lot because at some point in renewing my license someone got fat fingered and made a 1year incorrect birth date that I must go to the parish court to correct.

The fact your wife was able to get the check implies that there’s no issue with your account, or there was and it’s been resolved.
So they should be able to tell you what the issue was, whether it was an issue with your identification itself or the account. I’d ask to make sure it doesn’t happen in the future.

could be something like a DMV flag from not turning in a license plate from a previous vehicle. I had that one time. I turned it in and kept it but somehow they didn't mark it as such. Also, had a ticket that was reduced to another type violation and it never showed I paid it when I went get license renewed. It's that kinda stuff if I had to guess.

unless your driver's license number changed (opened with a different state DL), there should be no issues. Yes, I work for a bank.
