Anyone Seen a Juror Questionnaire before?

I got this in the mail today, just seems fishy. Plenty of "must return" and "mandatory" but it was sent via snail mail, no certification or anything. Have y'all seen anything like this before?

As slow and shitty as the USPS has been lately, I'd run it through the shredder and say I never saw it on the off chance that it was real and anything ever came of it.

In Lafayette Parish, if you’re selected for the pool, there is a much more detailed questionnaire. This looks like you’re about to be in a pool, but a phone call to the clerk wouldn’t hurt if you’re nervous about it’s validity. You won’t be the only one.

Looks pretty much like what I got in EBR. I filled it out and got called for jury duty several months later. I got dismissed though.
ETA - I think mine had an online option. I did not fill out the paperwork specifically.
