A mixture of not caring, ignorance, laziness, and weak wills. To what extent it is depends on the individual.
The fad diets are widespread because people are looking for an easy way out. They want to at least give the impression of wanting to improve themselves without actually putting forth the effort.
ETA: Also, I'll give a bit of information about my attitude towards nutrition. I eat what I think tastes good. It just happens that I like variety, and I can't stand eating crap all the time. Much fast food is disgusting to me. I don't avoid it because it's unhealthy. I avoid it because I don't think it tastes good.

Well, you're at Subway too.
I ordered a 6 inch sub, no chips, cookie, or drink. I also don't eat Subway every day and realize that it's not exactly the healthiest. I eat it when I don't have time to prepare my lunch, and choose it as it's a quick option and the healthiest available to me. And I workout 5 times a week, with pretty intense exercise, so it doesn't even effect me that much.

Not realizing that footlong sandwich she just ordered is probably over 1500 calories before she adds the chips, coke and cookies.
Well a loaded footlong Chicken, Bacon & Ranch sub on white bread with cheese, mayo and ranch dressing has 1140 calories so youre a litte off, but I get what youre saying.

This is true. You don't have to be an idiot, to believe Subway is healthy. It's heavily advertised as so. I would think uninformed would be the better word to use for someone eating Subway to "eat healthy"

Krispy Kreme is not advertised as being healthy.
Well she is even more dumb for believing advertisements.

1140 calories
That's another thing. Calories are not the end all be all when it comes to eating healthy. They are useful to look at and track, but all calories are not the same. Eating 2000 calories a day of sugar and processed foods, does not yield the same results as 2000 calories of natural foods.

It's close, regardless and that's before the cookies and chips.

I think the reasons are thus:
1. Eating unhealthy is generally easier and/or cheaper than eating healthy.
2. There are more options for unhealthy eating than healthy eating.
3. Ignorance as to what is healthy and what is not.
4. Lots of DGAF.
Agreed. We've become a culture that embraces convenience above all else.

Biggest issue is a completely warped view of portion size
Bingo. Most people don't get that even eating "healthy" doesn't mean you can eat as much as you want.

Embrace the heftiness, big women will shade you in the summer, keep you warm in the winter, and we all know they give the best dome polishes, (why because they have to)

That's the biggest past of the problem IMO....people in america are too easily fooled by marketing.
People are just sheep and buy what they're told to buy, as long as it is socially "acceptable".

well her meal was probably much better than two little ceasar's large pep pizzas.

It wouldn't be as bad if we didn't live such sedentary lifestyle now. Whether work or for fun, most people are sitting behind a computer screen and not doing manual labor anymore so they are not burning any calories during the day.
About 10% of the problem.

It's not just the US. It's global. The rest of the world is just catching up to us.

Has anyone mentioned stress yet?
After 10-12 hours of dealing with idiots and their inane "deadlines" I can shove back a 14" pizza and sixer of red and whites pretty easy.

Well that's kind of your own fault for working a job which leaves you such a poor quality of life. And its certainly your own fault for getting home and over eating/drinking.

Subway CAN be healthy. It's all about what you order.

I think it's generally a problem with the uneducated. That's why it's a bigger problem in The South and among minorities. This is a general statement, of course.
