Why would anyone leave the safety of the hospital to just wander around? Of course, Rick doesn't know that the church is packed with walkers...

I think a lot of the people, that weren't cops, were sort of prisoners there. I'm sure a few of them would rather leave than serve the cops..

Ok, I went back and rewatched that scene. You could see that Dawn had her weapon holstered when Beth walks up to her. She doesn't make any movements that would make it seem like she drew it before Beth stabbed her. As soon as Beth stabbed her, Dawn blew her brains out. When it went to slow motion, you could see Dawn mouth "No, I didn't mean to".
I'm pretty sure it she drew and shot her instinctively when she got stabbed.

That's what I was thinking. But i can't imagine that the culture of the hospita lwill change with such a rigid pecking order for so long .
