How about in Roseanne when Dan finds out that his sis-in-law is getting beaten by her bf, so he grabs his jacket and makes an excuse to leave. Comes back with a swollen fist. Gotta love John Goodman.

one of the first couple seasons of ER had a show where the wife of an elderly couple was dying. she dies, her husband (john randolph) holds her hand and sings a song while whole hospital watches. he kisses her and slowly walks out

Pretty good list by alajones. I agree with all but the last two.
Any episode of a show still in it's original run (not re-runs) should be automatically disqualified. It is still too recent to be deemed "epic."
Here is an episode that was so epic (in a bad way) that it coined a phrase:

Although it should be disqualified, there are plenty from House.
Wilson having to take April off of life support, or House hallucinating hookups with Cuddy and being totally out of his mind to name a few.
