In Sixth Sense there was no reason to wonder about Bruce Willis until it was suddenly clear.
How so? He was shot in the gut (fatal in a lot of cases) in the beginning of the movie. It was suspected he was dead after that, especially when no one would talk to him except Cole

I actually agree. That is one of my favorite twists ever even if all the sequels were a pile of dogshit that made no sense.
But “Rosebud”... I’m not sure if that classifies as a real twist. It’s a catalyst for the plot. I mean, after watching this whole movie about an entire life the man resents, it’s the only obvious conclusion that he died thinking of the time before everything went to shite for him. OK, it’s the sled. Fair enough, but I don’t even find that to be in the debate for best twist ever. It doesn’t change how you actually view the film since you knew he was an a-hole largely corrupted by the system before.

For me Sixth Sense and Usual Supects are top. I would add Jacobs Ladder too.
And yes...Angel Heart has a great story.

I would add Hitchcock's Psycho and Nolan's Memento on this list as well.

I wouldn’t say these were the greatest, but the multiple twists in Blade Runner 2049 were fantastic.

Shutter Island
I watched that movie while my wife (no pics) studied at a nearby table. On the boat ride out to the island opening scene, she called the premise of the movie only hearing the dialogue. I’m sure others also figured it out easily too.
The Sixth Sense is #1.
Brad Pitt has some good movies. But unless the question is “what are the top Brad Pitt movies?”, a movie list should never start with two Brad Pitt movies.
“Rosebud...”
Peter Griffin: “It was his ( )! I just saved you 2 boobless hours.”

All the best ones already listed in this thread. A couple honorable mentions that haven't been mentioned, though - Primal Fear and Gone Baby Gone.

Yeah there were a lot of clues...
It wasn't just the gut shot...but you knew he was shot...for me it was the throwing the rock through the window and nobody reacted when he saw his wife with another guy - that told you everything and then the dinner scene confirmed it.
Wait...he's treating a kid who sees dead people...and no living person who is supposedly close to him reacts to him on a daily basis?
M. Night definitely put in clues.

I just don’t look at a catalyst as a twist. Regardless if Bruce Willis was dead or not, Haley Joel Osmont was still seeing dead people. That’s a fantastic twist that he was dead the whole time and we didn’t know it. Rosebud isn’t a twist as far as I’m concerned. It’s the natural conclusion. You could have ended the Sixth Sense differently, and have been a great film, but the end brings it into a horrifyingly different context.

That his dying words were about a stupid sled.
(signifying youth and lost innocence)
You think it's a woman or something about the entire movie.
