1998 vs Michigan State was brutal because we hadn't really sniffed a title since 1970 (doesn't count even though Ohio State recognizes it).
We were full on Joe Biden sniffing titles in the 90s and couldn't get over the hump.

The answer has to be WVU. Losing at home to your archrival (with a terrible record btw) to prevent you from playing for a title is the stuff of nightmares.

It would have been slightly less surprising if Pitt had won something like 34-31.

Here are some honorable mentions. I may add more as they come to mind. These programs have recent titles to ease the pain, but the losses were unbearable for their fans at the time:
Ohio State 24 vs. Michigan State (1998)
Ohio State is a blue blood, but they hadn't won a title in 30 years at this point. John Cooper had many great teams that fell short throughout the 90s, but this was his best one. The wire-to-wire #1, Ohio State was favored by 28 against a 4-4 Michigan State team. They got out to a 24-9 lead, but momentum shifted once a shanked punt by Michigan State hit Nate Clements in the back and the Spartans recovered. Ohio State completely unraveled after that, their last-minute drive to salvage the game ending in an interception. It was so bad it made Neutron Man cry.
Georgia 28 vs. Alabama 32 (2012/SEC Championship Game)
Basically the real national championship game that year as Georgia would've crushed Notre Dame just like Alabama did. This art sums it up. Georgia was up 21-10, but Alabama came back to take a 32-28 lead. Aaron Murray led Georgia on a last-minute drive. It seemed like Georgia would get a last-second TD, but Chris Conley caught a tipped pass inbounds and the clock ran out. This was Mark Richt's best chance at a national championship.

Pitt shoved 9 people in the box and Rich Rod was too stubborn to change things up. Then compound that with opponent having a terrible record so the guys were overconfident, MCafee missing 2 kicks he almost never missed, then Pat White being injured and out for about a quarter.

As a teenager I felt bad for Carolina fans. It wasn't a bad loss it was a beat down.

The 1st kick was a 19-yarder from the left hash. WVU intentionally took a delay of game penalty to get a better angle for the kick, but Pitt declined it.

But I'm sure USC fans would hold 4th and 2 in the 2006 Rose Bowl as the biggest heartbreak based on historic implications on the line, as Carroll said after the game..."we would've been the be all and end all." First team to ever accomplish the feat.

Michigan lost at home to Oregon 39-7 the next week after Oregon took their foot off the gas at halftime. While embarrassing, that App State loss didn't really cost Michigan all that much.

Really surprised LSU vs Tenn 2001 isn’t on there. Arguably one of UTk’s best teams all time loses to a backup QB in the SEC championship game. That eliminated them from playing Miami in the NC game. Miami went on to blow out Nebraska for the natty

In 2014, TCU gave up 24 points and blew a 21 point lead in the 4Q against Baylor.
Only loss on the season was to #5 Baylor on the road. Finished 11-1 and was ranked #6 at the end of the season. Pretty harsh and 12-0 would have certainly secured them a spot in the playoff. Ohio State ended up jumping Baylor and TCU to take the #4 spot in the CFB playoff and then beat Alabama and Oregon to win it all.
