you do, but you still have to at least get to quest where you kill his wife then he starts working

See this is what I was talking about the other day.
My blacksmith is there in New Tristram right when I start Nightmare, shop set up and all. I can't do the quest to kill his wife again. It never comes back up.
And to make it more confusing, when I start a completely new character, my blacksmith is there. Shop set up and all. Is blacksmithing/jeweler supposed to cross all characters? Do you just level them up once and it counts for all your characters? That seems weird.

Yep it's for all your character and even though he isn't there when you start a new difficulty you should be able to click any of the BS structure to open the option for the blacksmith.

Killing his wife is one of the main quests, I've done it on multiple characters on different difficulties.
