My Nonna is Sicilian and we had boiled eggs in red gravy every Sunday when we were growing up. I continue the tradition and always add them.

I don't see how anyone would think this is gross? I've never had it, but egg white is pretty tasteless and the texture is soft enough when mixed it would almost disappear I'd suspect.
Eggs were always around 100 years ago because everyone had chickens, fresh meat was a different story before refrigeration. You either slaughtered something, had salted/ preserved meat, or didn't have meat.

So at what point to you add the egg? Is the egg first hard boiled and then at some point thrown in with the sauce? Or do you throw the egg in uncooked and let it cook with the sauce in the shell and then peel the shell off later? How does this work?

Put it in peeled & already cooked. You don't want to be picking out shell.
