What happens if you refuse to pull forward in a drive thru?

Been hearing that fast food joints have been doing this a lot in recent years, to scam the system to show that they have fast service.
If they ask you to pull forward and you don't, what happens?
EDIT: Some drive thru window workers here are triggered

I have a friend who worked at McDonalds in high school. He used to tell me horror stories about how when customers were assholes when ordering or at the window, they legitimately would spit loogies into their food.

I refused to pull forward years ago. They just fussed at me and finally brought my food. I've always felt bad. It's really just selfish not to pull up, even though it's hypocritical of drive thrus to ask it.
