Am I seeing this all wrong? Is this his tie tied around his neck but inside his shirt? Or is this something genteel and over my pay grade?

I think he always wears that. I think it's an ascot, but he uses a necktie to make it. First noticed it a few weeks ago. He took the oversized tie knot to the next level.
