As you didn't specify lunch or dinner, my suggestion will be for lunch, just because I haven't been to a traditional, extremely elegant Italian restaurant in the city since La Riviera.
My favorites are Liuzza's on Bienville and Red Gravy on Camp St. These offer traditional Italian fare, but may be a little more casual than you're looking for but you won't be wasting your time or money, I assure you.
If you want dinner, I would suggest Mona Lisa on Royal Street in the Quarter. The food is very good but it's not a formal Italian restaurant.
You might also check out Domenica. It's in the Roosevelt Hotel. It's not (in my opinion) a romantic Italian restaurant, but the food is excellent and the Roosevelt Hotel is amazing.

Irene's gets my vote. Italian Barrel is also great.
I personally think that Cafe Giovanni is extremely overrated but a lot of people like it.

I haven't branched outside of Cafe Giovanni, but all my experiences there have been great.

Definitely not your standard red gravy Italian place but I thought AVO was great.

I like Italian Barrel and Marcello's. Not a huge fan of Cafe Giovanni.

My buddy went before a Saenger show recently and he was not impressed. He said service, food quality/taste, and value for the $$ was not what he expected.

Red gravy is excellent and another vote for Domenica. I'm more of a fan of traditional italian than that bastardized creole italian that new orleanians seem to love.

I like Impastato's but it is in Metairie. If Uptown I would go to Vincent's on Carrollton. If downtown, I would go to Domenica or Café Giovani. I made the mistake of recommending Andrea's last week and got ripped for it, but they have a 31st anniversary special going on right now. Three courses and a glass of wine from Andrea's vineyard in Capri for $31.00. We had it last weekend and enjoyed it very much.
