I remember an interview with a former Medicare upper management type who said back then that he estimates at least 25 percent of all Medicare payments are bogus in one way or another (fraud, overpayments, double payments, payments to deceased people).
He claimed it would cost too much to go after the fraudsters, so they let it ride.

Medicare fraud used to be treble damages if caught. Not sure if that is still in place but fearing treble damages should be plenty to scare someone to death. I bet the most is in home health and upcoding in doctors' offices.

This is going to be a lot tougher because in a sane world it’s all going to be payments to healthcare entities - so you have to show payment isn’t for services provided.
Of course with our Govt I won’t be surprised if billions aren’t going to NGOs and foreign govts right under our nose.

The Medicare and Medicaid fraud they do catch is obscene. And that’s only about 5-10% of what occurs.
