Hands down, worst Southern accent?
An obscure independent cheapie from 1933, entitled "Her Secret."
This film stars European actress Sari Maritza, who had come over to America and made a film or two for Paramount, which apparently didn't make any kind of splash. So she was then given a job as the star of a little independent production shot in Arizona.
In the films she plays a Georgia girl, who runs a little roadside diner that caters to the nearby college crowd. Her attempt at a Southern drawl is jaw-dropping. She talks slowly and lowly, and comes across as if she's brain-damaged. Ghastly. You can't help but laugh at every line she utters.

I thought Alec Baldwin in Ghosts of Mississippi was pretty bad but it's been a while since I've seen it. I watched the trailer again and maybe it wasn't that bad. That was such a good movie. James Woods was so good in it.
Agree that James Vanderbeek in Varsity Blues is probably the worst.

not the worst but one that doesn't get mentioned often is Paul Newman in The Long Hot Summer. Joanne Woodward's accent in that movie was pretty awful too.

It is a bit sad that when you need an actor or actress to nail a Southern or Texan accent and you can't find an actor from that region... you're better off casting someone from Britain or Australia than most of the US.

Charlize Theron in The Legend of Bagger Vance. It sounds like a bad imitation of Vivian Lee from Gone With the Wind.

The Southern accent is closer to the Queen's English than any other.
