Based off biology. We as humans weren’t really on the gender is a social construct when male and female roles were being formed in the beginning.
Think about it. You don’t even have to have a shitload of knowledge to understand. In ancient times why would it make sense to send the breast feeding woman out to hunt and leave the man that can’t physically nourish the child at home? It’s common sense mate. It’s all based off the biological differences between the sexes.

We can still think a woman can look butch, and a man can be girly. Someone doesn't have to be female to be feminine nor male to be masculine. And what one culture may think is feminine can be considered masculine in others.
I'm not saying trannies don't have psychological problems (often involving how they perceive society as seeing them), but there is a difference between sex and gender.

I vaguely recall a noted psychologist or psychiatrist wrote a book about transgenderism is a mental health issue. I googled it and looked for it on Amazon. No luck.
