When I first became a parent, this was a fear of mine as I seemed to see news stories about it happening all the time. Several years later, I now don't know how that possibly happens. The kid should be the first thing removed from the car.
edit: and before the naysayers come out, in all of these cases, you'll probably find that they remembered to get their phones and wallets/purses.

This. Or those who are completely ignorant on mental illness and the way it can affect one’s thought process. I do not condone suicide but this is a situation where the pain he felt was probably unbearable. The person I feel the worst for is the wife.

I gotta say, I can't say I wouldn't probably do the same thing if my actions directly result in the death of one of my children. There's no possible way I'm overcoming that.

example: if he rarely or maybe even never drops kid off on way to work in morning. normal routine kicks in, mind wanders, etc. dad literally does exactly what he does every morning without ever even thinking. baby doesn’t cry/make noise or anything just adds to it.
not excusing him in any way. just one of the more common threads in these incidents.

As always we need more facts. If alcohol was involved, I can understand what happened (though not necessarily excusing it).

It’s pretty rare. You explained how in your post you do the same thing by going on autopilot and performing your routine even though there are no children to drop off. That’s how these things typically happen, but a bit of the inverse of what you have experienced.
