Are these really an issue, or do restaurants get violations all the time?

Seriously though, every restaurant has health code violations. It's not a question of if. Here are the questions relevant to a health inspection:
When did the health inspector happen to pop in for the test? Was it during the busiest time of the day or was everyone just standing around looking for something to do?
Was the restaurant short-staffed at the time?
Does the health inspector have a good relationship with the restaurant?
Can the manager stall the inspector long enough for someone to dump any food recently placed into a cooler?
How often does the health inspector eat at the establishment? Does he/she get special treatment when dining there? (the answer is yes, even if they don't realize it)
