My guess
O2 tank blew, while plane still climbing (engines full throttle).
Explosion and concussion knocks the pilots forward and probably unconscious, driving the stick forward and down.

If an oxygen tank was defective, would changes in pressure potentially cause an explosion, leading to a crash?
