At 31%, North Korea has one of the highest starvation/malnutrition rates in the world. From reports from numerous defectors and undercover investigations, the "happiness" is a facade generated by implied and/or overt threats for the people to appear happy to the outside world or face the consequences of violent reprisals against them and their families.
Or perhaps they were just promised food and that made them happy.
