this event has been the "wake up call" for you, but fortunately, it probably hasn't debilitated you or done you in, like it does to so many.
You are absolutely right, weight loss is a must, but I'd like to re-word it for you, that a proper diet is a must. The weight loss will reflect the proper diet.
We can't change your DNA, so people need to do their best at changing or maximizing their behaviors.
No tobacco
Alcohol in moderation.
Exercise 150 minutes weekly.
Proper diet
I have been encouraging the Keto diet, and have seen (anecdotal) evidence of:
Reduced blood sugars (obviously)
Significant weight loss
Cholesterol improvement, specifically Triglycerides and LDL
Blood pressure improvement.
Arthritis pain reduction
If keto is difficult to organize, just avoid these big five
Sugary drinks and snacks which are obvious. -Sodas (diet too), fruit juices, gatorade, candy, ice cream
Potatoes
Rice
Pasta
Bread

I have friends who are internists who take low dose Crestor or Lipitor prophylactically. It's supposed to reduce inflammation.

Not sure if she's right or wrong, maybe she has some evidence to support her claim. There is still something causing the specific arterial inflammation, whether it's from particulates or something triggered by poor diet, stress, sedentary lifestyle, etc. The key to minimizing the damage is identifying the origin/catalyst of the inflammation.

that is my concern also. they say heart disease can be caused by inflammation due to stress and smoking. i do not smoke but i do have stressful ongoings at work and home. i had my CRP c reactive protein checked but my MD could not tell me if it was bad or what.

Other than tracking weight loss, get blood work periodically to see how your numbers change and use that to refine your diet.
Like others have said, try to cut out as many carbs as possible. You might need to ease into it but intermittent fasting has worked great for me. I have lunch between 1:00 and 2:00pm and don’t eat again until breakfast at 9:00am the next day. Coffee with some real butter and heavy whipping cream helps. Don’t kid yourself that natural sweeteners like honey are much better for you than refined sugar - just cut them all out.
One tip I got on the Food Board was about Mission brand low carb tortillas. I use those to make lunch tacos with baked chicken breasts or high quality ground beef, salsa, and a ton of shredded lettuce.
If you have a setback, don’t get discouraged - just get back on the plan.
