One of the issues with the cameras onboard the rocket is that the vibration is very difficult to isolate. I worked on this issue for a year with JPL and Boeing. The communications connections between a COTS camera and a COTS lens are pretty solid until you apply huge vibration oscillations and g-forces. If the shakes loose it can come back or not. We worked on the "or not" issue.
On the ground, that was surprising. That system has a laser designator for smooth tracking. The team responsible for that, on the most important launch in a long time, have got to be embarrassed that they screwed that up.
