Anybody who marries someone from the same small town is almost guaranteed to be related to them some way.

No. The opposite it true. For every generation of first cousin marriages, you can deduct 6 points from the IQ scale.
In Muslim countries it is OK to marry first cousins because Mohammed did it. So about 30% of all marriages in Pakistan are to first cousins. And look at how that is working out for them.
In the UK I read that Pakistanis represent 2% of the population but their babies represent like 20% of the birth defects.
It's gross.

In the Middle East, marrying first cousins is part of their tribal culture.
In Louisiana, first cousin marriage is illegal.

If you go through history, you will note that an uncomfortable number of marriages were between second cousins or closer. That's just how it was. Only in the last century or two has that trend started to go away.
