The average board scores for a neurosurgeon are far greater than the average for general surgery which is the path to take to become as you mentioned a heart surgeon, vascular surgeon, colorectal surgeon, surgical oncologist. The average Johns Hopkins neurosurgeon is exponentially higher than the average neurosurgeon. So that covers the question of other medical professionals.

Well first of all the herd gets trimmed in pre med. There are multiple weed out courses containing high levels of biology, chemistry and physics. Then there is the MCAT which if you survive pre med you might not survive that. Then there is med school which as above. Then it is minimum 7 years of residency making crap pay and in a research intensive field at a research intensive institution such as hopkins you are the leading innovators in medicine. I think that qualifies as more brilliant than the average person.
