What is the proper way for a man to shake a woman’s hand?

It always seems awkward when you have to do this in a work setting. One time I shook a woman’s hand firmly, and she got offended and looked at me like I was an idiot.
Another time when I shook woman’s hand, I grabbed her hand softly and she grabbed mine really firmly, making it look like I was a little weakling.
