It was very common of the generation before me, parents and their peers, to be called by middle names.

I think this was a traditional thing.
My great grandfather was a twin and they had the same first name. Different middle name.
Also, back in the 1800s it was very common for cousins to have the same first and last name. Assuming it was always like this until recently. Maybe this is where the middle name comes in?

Back when everyone was named John James and Thomas middle names sure helped out a lot.

My Italian boss has no middle name.
I knew a French husband and wife who had the same first name, so subsequently shared the same name, and their middle name was the only defining factor.
