re: What in the hell is wrong with medical billing?

Many have coinsurance so the final bill isn't known until services are rendered, insurance is booked, and it spits out what is and is not covered. Insurance will also say if deductible is satisfied or not. This is why people with coinsurance don't make payments before services (except for some stuff like surgery that a deposit is collected).
This is laid out in insurance"explanation of benefits" explaining each code charged. You'll either getting it in the mail or it's on the insurance website. Everything is billed by codes so it'll tell you what codes are covered and how much insurance paid and how much you owe. You can google the codes to get an idea what was charged.
I'm not saying it's all easy, just that the info is usually there if you know where to look. No one should be in a position that they're being charged for something that can't be explained. If that's happening then, yeah, I'd fight back on that too.

There’s no other explanation in my case. Run of the mill insurance, gets bill for miscellaneous menial amounts after already paying a copay at time of service.

And thanks to shithead John McCain who cast the deciding vote by making a spectacle of himself to kill the bill that would have buried obamacare forever..
FIFY.
It wasn't Trump's fault, it was traitor McCain.

You can honestly thank both the Democrats and the Republicans who have made this a partisan issue instead or working together to craft a real, meaningful solution. Of course, the real reason they haven’t is that would benefit the people they are supposes to serve, instead of the corporate overloads
they really work for.

Dentist here and I agree that it is a total racket and it makes me feel like I'm a con-artist or something sometimes. The worst is when we get a pre-estimate from the insurance company for a procedure to get an accurate OOP for the patient, and then after the procedure is done, the insurance changes their mind and says the procedure wasn't necessary and we either need to be the bad guy and bill the patient more and run our reputation and business into the ground, or write off the loss and run our business into the ground

Neither party has an incentive to fix it. By leaving it broken, they can continue taking their lobbyist payments from the insurance/pharmaceutical industries while campaigning on "lower healthcare costs for everybody!" to fire up their bases. Knowing they won't do shite and can blame the other side. Whole thing's a scam.
