If the trans person starts hormone therapy during adolescence, the bone structure will usually closely match the self-identified gender, so you can't always use that to tell the difference.

Actually sexual reassignment surgery is so good these days that it's hard to tell by looking downstairs if it's a male-to-female transition and the transwoman got a good surgery. Not all of trans people change their genitals.

I care about trans issues because I have a few friends who have come out as transgender and subsequently transitioned after spending a lot of time in therapy and consulting medical professionals. At first it was difficult to understand and I had a lot of misconceptions about trans issues, but I put in the effort to learn about it because I care about my friends.
