That tornado was an extremely violent EF-5. We know that it scoured the ground and pulled pavement off the ground. So, I think it’s possible that it could have done that depending on the construction of the shelter, the door on the shelter, etc.
The Hackleburg/Phil Campbell tornado was the deadliest single tornado of the 2011 Super Outbreak and is the deadliest tornado in Alabama history. It killed 72 people.

I tend to believe it after reading alot.
The Smithville, MS EF5 pulled an iron drainpipe buried 8-12" deep from the ground. There are pics to support that. It also pulled, I believe, a water meter box from the ground.

it had warnings on it almost continually (there were a few miles on either side of Louisville where it wasn't warned) until roughly the Kentucky/Ohio border near... say... Maysville, Ky.

People saying that you have to be below ground: look at the research from Texas Tech’s wind institute. With the help of FEMA, they developed above ground shelters that can basically withstand anything.
I saw multiple instances, while doing clean-up work in the days after the Moore 2013 EF5, where the entire house was gone except for the above ground shelter.
My parents have one, and you can’t believe how thick the doors are on these things. It’s pretty wild. I’d absolutely feel safe in there no matter what was bearing down on us.

My aunt lives north of OKC and they have one thats above ground. I dont want to imagine the tornado it would take to damage that thing.

I know a guy who builds/installs both residential and commercial (for towns and cities mostly) above ground shelters. There's alot to meeting the standards.
Some neighbors of mine did a closet/safe room in their home when they did an addition that would be ideal for new home construction. Inside it just looks like a closet. I have a full set of plans for one like it kicking around somewhere.
Those type shelters can be added to an existing garage or building that is on a slab. It is getting more and more affordable every year it seems.
Whatever option anyone chooses, a helpful bit of information is that you can register your shelter's GPS coordinates with FEMA and local EMAs. Look at the wreckage we are seeing now. Finding shelters in that can be difficult. You often lose cell service after a tornado like that. You can be trapped by debris in your shelter, but otherwise be perfectly fine. Having that reassurance on file can give a little peace of mind if such a disaster strikes.

Funny you say that about registering the shelter: the steel is so thick that you can’t get any cell signal inside it with the door closed.

A relative of min had an in ground shelter, which is pretty rare for Louisiana. His wife took one look at it and said she'd rather take her chances with the tornado than in there with the rats and shakes.

If that is not from Tuscaloosa 2011, it sure looks similar.
I'd seriously consider installing one with a new construction.
But, with all the rebar and concrete and steel, I'm sure it is very costly to add, even for a small hall closet sized one.

Read a story somewhere. A family just moved into their new house and were excited about the kids having their own bedrooms for the first time. The 9 year old kid died and everybody else had to be airlifted to the hospital
