I never quite understood this. Regardless of his mental function, he used a gun to kill people. I don't see why we adopted this attitude that he didn't mean to do it. So now versus just executing the guy, he will spend the rest of his life in a mental hospital paid for by taxpayer.

A Sanity Commission finding someone incompetent to stand trial doesnt have anything to do with the person's guilt.
It's about if a person understands what's going on in the proceedings so that he can assist his attorney in preparing his defense.
The court generally assigns two doctors to do an evaluation. If the doctors say the person cant assist in his own defense and doesnt understand what's going on, then the judge will usually go with their conclusions.
At that point the person is shipped off to a prison psych facility until they are "rehabilitated" and can then stand trial.
