It also can be successfully used to correct a previously failed cartilage repair surgery. Freshly obtained grafting material, as opposed to frozen or freeze-dried specimens, is used to help ensure the best outcome. Ideally, the graft material is taken from the same location as the patient’s defect and is roughly the same size.

To perform this procedure, the surgeon makes an open incision and identifies the defect. Once the size has been determined, the surgeon removes the abnormal cartilage and a small amount of bone, creating a tunnel. Then, the fresh donor material is obtained. The surgeon attempts to obtain donor material that matches the size and shape of the patient’s tunnel. The donor material is then inserted in the tunnel.

Rehabilitation

Post-operative rehabilitation consists of toe-touch weight-bearing for 6 to 12 weeks. Early progressive motion is encouraged with a special machine. Some surgeons recommend limiting bending the knee to less than 45 degrees for the first 4 to 6 weeks for some patients. High-impact activities, such as running and jumping, should be avoided until 6 to 12 months after surgery.

Outcome

Osteochondral allograft transplantation has been performed for the past 50 or more years with several studies demonstrating satisfactory outcomes. In 90 percent of patients, grafts survive at least 10 years. At 20 and 25 years post-surgery, grafts remain in up to 70 percent of patients.


Anybody have this done? Sounds like a serious pain in the arse for a year at least. Should I just wait for knee replacement in about 10 years?

I’m a pretty smart guy, but if you gave me a piece of paper with this on it and a gun, and I couldn’t leave the room until I came up with an acceptable definition, I’d last 5 minutes

June 8th I had MPFL reconstruction
I received a cadaver tendon & it was attached to my existing tendon as reinforcement

I will say I have a very, very high pain tolerance
They sent me home with morphine to take every 6 hours & Percocet for breakthrough pain
Had surgery at 3, home by 6, took a morphine in case I started hurting
Didn’t take more pain meds until a few days later
Since then I’m only taking a Percocet at night, only to be able to sleep because the brace is the most uncomfortable fricking thing ever

When I went to my 2 week post OP the PA said as long as I could walk normally (as normal as you can in a straight leg brace) I could stop crutches

This week my PT said I could stop wearing brace around the house & at night

Mine was my right knee, I don’t think I’m technically released to drive until the end of July

PT is 3 times a week for at least 3 months

I’m just now able to bend my knee about 70 degrees

If you can wait until the weather is cooler I’d recommend doing so, it’s miserable with the brace

My mom had a cartilage transplant years ago but they took hers, grew it in a lab for a month then put it back in her knee
It’s been a long time ago so I don’t remember specifics

You have to rehab like you would any serious knee injury, like ACL etc. Whether or not you should wait depends on you, your age, and many other factors you should discuss with your doctor. Modern implants are made to last 25 years or so, depending on wear. Second implants are more technically challenging for the surgeon due to less bony material being present.

The OT is definitely where I come for advice regarding very sub-specialized orthopedic (or Orthopaedics for those across the pond) surgery treatments and expect to get legit answers.

I know the OT BS. But there are a few who have valid opinions. Have to wade thru the BS.

Doctors opinions are NO different. I wanted a "gel" insert, but told no. Going for 2nd opinion.

Olympic lifting, running stadiums for 20+", squats, step-ups, heredity of bad joints in the entire family.....yeah, I'm f'd. I only do recumbent bike and dynamic (Sprints) workout, no impact on joints nowadays.

60% cartliage loss, bone on bone when walking to mailbox. shite hurts.

I know one woman that is a semi-pro triathlete and she had to have this done - she's still doing Ironmans and seems to move fine. She also finished an Ironman with a broken toe though - so she's kind of a psycho lol. It still sounds miserable - best of luck to you!

From studying all the ways your body can reject a foreign body part I just can't imagine putting someone else's body part in my body unless there were no other options.

I asked if my cadaver tendon was “fresh”
It’s not frozen but it was “processed”
Your body sees it as a medical device, not a living part of the body so there is basically no chance of rejection
I received some paperwork with a code, if I want to write a letter to the donors family I send it in with the code so they know who to forward it to

I’m a physical therapist in New Orleans and I will just comment that I have never seen one of these cartilage transplants actually go well, and every one I have ever seen has wound up having a knee replacement. Can I ask what surgeon you’re looking at?
