I have a Ph.D. and work with actual medical patients in a hospital. I’m a doctor there but not outside of that context. I will also clarify to patients that I’m not a “medical doctor”.

You’re on a plane and start feeling bad, have intense abdominal pain, and pass out. Your wife/husband/significant other calls the stewardess over and she makes an announcement over the PA, “Is there a doctor on board?”

FYI physics, math, biology, and chemistry are all liberal arts subjects.
It is interesting that 90% of people who lambast liberal arts aren't educated well enough to identify what subjects are actually liberal arts.
However, fewer don't know how to capitalize the abbreviation for Doctor of Philosophy and fewer still don't know the difference between possessive and plural.

If they can’t handle an emergency medical case, then they shouldn’t be be in the medical field. It is not like they are going to need to perform open heart surgery at 10,000 feet. All what they need to be able to do is stabilize the patient and hand them off to a real doctor on the ground like they typically do in a medical emergency.

The choice also has a lot to do with the specialty of the doctor. If I have a heart attack on a plane I would rather have a cardiac RN that regularly does cardioversion and is at minimum trained to do AED defibrillation using the onboard AED than an orthopedic surgeon. That preference also applies to a paramedic that is trained and used AEDs and manual defibrillators.

Everyone who earned a doctorate is fine to call themselves a doctor in their professional setting. Outside of their professional setting, they are just douchebags if they want to be called that. Someone used the what if a medical emergency on a plane and asked for a doctor, that is clearly a situation where it is the professional setting of most MDs.
