The “something” you are thinking of is cytokine storm.
It’s also associated with smallpox and is suspected to be the primary cause of death of that disease (they honestly don’t know how smallpox kills).

If it mutates and can pass person to person it will easily be that bad if not worse. There has been many simulations done on this and it is quite scary. There are over 3x as many people today then in 1918 and you can cross the world in 24 hours. It's not a if but when.
