Analyze the reward systems (incentives, promotion, recognition, rewards, reputation) and you will likely find much of explanation for human behaviors.

This part, from the 311 page report which I've yet to link, seemed to be due to subcontractor issues. Which confuses me... how a subcontractor has, I guess, "privacy rights" in a governmental contract.

Contractors and their subs can use proprietary materials, etc in their design. NASA will respect that and not disclose it publicly.
Issacman's letter was good. Very good. But at some point, he's gonna have to confront DEI within NASA.

IDK if there's an easy way out or a clear path forward. Most of those contractors are also eaten up with DEI. Yes, NASA could do that, but who would be left to submit design proposals to NASA?

It's almost like someone realized that if NASA doesn't get their stuff in line they're going to get lapped by other countries and or SpaceX/etc. Also it should be painfully obvious the whole starliner incident was a gift to Boeing considering someone actively pushed along a product that should never have been greenlit.
NASA is now just a contracting agency outsourcing everything and blowing through taxpayer dollars to favored defense contractors. IMHO SLS should never have been allowed to move forward...
