LL Bean.

When I was in college I bought a pair of LL Bean shoes at the beginning of the semester and they pretty much fell apart by the end of the semester. Now, I pretty much wore them everyday, but I still thought that they should have lasted a few more months. I really couldn't afford a new pair.

I called LL Bean (long before the internet) and, after checking on the earlier order, they sent me out a new pair no questions asked. They didn't even require me to send them back the old pair. The new pair lasted me about a year.
