Best to worst, most desirable to least desirable, smartest to dumbest, whatever criteria you want to use.

I infer that primary care gets a lot of docs who don't qualify for anything else, which sucks for the patient, because you need somebody good who knows the difference between nothing to worry about and get this guy admitted stat. Hopefully there are some good people who go into it just because they like that kind of medicine.

I assume surgeons are at the top, and/or specialties where you get paid without having to actually interact much with patients.

Every physician I've shadowed has said Dermatology and ENT are the cream of the crop

Idk.

If I would have gone to med school, Cardiology would have been my thing. Still might get board certified in cardiology as a pharmacist when I finish school

No real way to rank the specialties.

So many different factors that come in to play.

Work hours.
Call schedules.
Respect in community.
Overhead costs.
Impact of physician extenders on pay.
Number of competing physicians in practice.

Sadly, most medical students look at the desirability of the position as seen by OTHER physicians not the need of the specialty in the community.

Problem with that is this.

Radiology - technology has changed it. Digital transfer of images allows competition ACROSS continents. Lowers demand and pay.

Anesthesiology - Nurses have entered the fold. Lowers demand and pay.

Girl I know is trained as an anesthesiologist, but runs a chain of medical weight loss clinics. I guess she makes a lot of money at it, but it seems trashy and a waste of a lot of schooling.

Is there a field that operates (no pun) in an inefficient market? I really think no. There are fields that see aspects of the specialty picked off (primary care/np/PA argument, optho has od's doing procedures in LA, dermatology vs board certified dermatologist, etc. Other fields have aspects that many students don't like. Some fields deal with lots of govt reimbursements. Pros and cons exist for each field.

No, students decide which specialty they desire based on what fits their personality, interests, and career goals. If I'm spending 200k on my education you better bet I'm not letting "community needs" choose for me. Nothing sad about this.

It very much depends on your career goals and personality.. dermatology is considered most desirable bc of it's best quality of life aspects (no call/flexible schedule) + great pay.

In most of the other specialties, the med student has the perception that they must choose between whether they want better quality of life or higher pay.

MrSpock's Ranking of Medical Specialties:

1. Whatever specialty best fits you and your personality.
2. The Rest

all med specialties can make money. I know some family practice guys that makes over a million a year, it all depends on how you run your practice.

dont worry about money, it wont change how you live your life, it will change how your wife lives (if youre a guy).

worry about satisfaction. Do a good job, money will come.

also, if youre worried about money, dont work for anybody else but yourself.

Why is this considered pretty much the lower end of the totem pole of medicine?

Average compensation for pediatrics is low by Doctor standards. I think the field is incompatible with a lot of physicians with shitty bedside manner too.
