Tell the Dr at the next appointment. More than once, I've told a doctor what inhouse imaging was trying to charge me for something simple, or similar, and they had no idea it was going on.

Exactly.
That’s why I don’t buy the “well it saves you from a surprise bill” bit.
A) it’s not a surprise and if it was you’d be an idiot.
B) I can set up a payment plan any way so what’s the difference between now or later besides the added headache of dealing with a refund.

This is how LWHA in Baton Rouge does it.
OB care is usually billed with a globalized billing model meaning all of the pre and post natal care is covered under the same cpt code for delivering the baby. That’s how they know how much it is going to cost.

Just do it. It is not a huge deal. And if you don't want to know the sex of the baby, they'll add in the circumcision fees and you'll get that returned to you if you have a baby girl.

I think its a good idea paying "ahead" of time.
and ESPECIALLY if your wife loves the doc etc. That in and of itself is worth any overcharge you think may happen. You change docs and your wife isn't happy or comfortable for 7 months and you will be begging to go back.

Alright think I’ve gotten to a better resolution with them. A lot of the issue was sticker shock over the amount they were charging. Looking into it there was miscommunication between them and my insurance over what would be covered and it lowered it a good bit back to something more manageable once everyone was on the same page. Still annoyed by the whole process but slightly less so now.

My kids are in their late 30’s and I had to pay a large amount up front during my pregnancies. They also required a $500 deposit for an epidural, so I didn’t have one for either birth.

Many insurers pay a bundled fee at the end, and that's assuming they actually pay, you don't lose or change insurance or even change doctors. Not unusual to get refunds after insurance pays but they have to ensure adequate cashflow to keep the clinic open to deliver the care needed and pay their own bills.

25 years ago, my wife and daughter’s bill was $750,000.
They f’d up the insurance for daughter and tried to say she wasn’t added within the allotted timeframe. She was in level 3, 2, and step down NICU for over two months. I called bs and showed them the paperwork. Fortunately it got fixed and we paid zero.

Completely common in OB practices.
1) when insurance reimburses the OB practice for delivery of the baby, it is a lump sum that includes doctor fee for delivery in the hospital, but also all 8-9 months of prenatal visits, plus some post partum care.
That means that for the 8/9months the ob is seeing your wife that they don't get paid squat from yoir insurance until after delivering the baby. They still have overhead/staff to pay etc.
2) as already mentioned, as you can imagine, lots of people get all this care, deliver, get the sent bill and disappear. And as if it wasn't hard to collect on bills before, now with the new law, good luck.
So a lot of practices will do a payment plan to collect over the course of the pregnancy to ensure they get paid something while providing care and then refund as needed.
