The amount of heroin he could tolerate was supposedly legendary. He had also just escaped a rehab where he landed after trying to off himself in Rome with a bunch of ruffies, suicide note and all. Give Disgraceland a listen.

Dude, obviously it was Dave Grohl who killed Kurt Cobain.
He needed him out of the way so he could begin his path to stardom.
Grohl would still be a no-name obscure drummer if it wasn't for him offing Cobain.

Seems odd that someone familiar with shooting up would “accidentally” shoot up that much.

There's a few other issues with his death. He was shot through the right side of his head. It may be possible to hold a shotgun up to the side of your head but it's certainly awkward. Going through the mouth or under the chin would be easier. Oh and Kurt was left handed so going through the right side is even more strange.
He had a little leather pouch that he kept his needles and stuff in which was all zipped back up nicely. Kind of strange considering the amount in his system.
As far as documentaries, check out one called Kurt and Courtney. It's much better than Soaked in Bleach.

He was a junkie. Junkies can handle outrageous amounts of heroin.
The human body's ability to develop tolerance is amazing. It's a big way people who clean up die. They are clean for months/years and then try a dose they would've handled easily before and they OD because tolerance goes back down to normal levels (somewhat).
He could handle enough heroin to kill several men. It has been written about a good bit.
