Requests to sit in on initial calls with client
Requests that all issues with client big or small be brought to his attention even if you have the capacity to resolve issue yourself
Sends customer satisfaction surveys upon project close.
Expects written/verbal explanation when negative comments are left on survey even to more seasoned employees.
Requests written next contact date in instances where project is placed on hold or initial call hasn't taken place.

Nope, my boss lets me do my thing. If I need him I go to him. If he needs something he comes to me.

Nope, talk to my manager like twice a month, and it’s usually just exchanging pleasantries. I know what needs to get done.

More than likely, he/she is a control freak and believes the company cannot operate without him/her being there and in charge.
What he/she fails to realize is that this type of managing style causes more people to not want to invest back in the company and not willing to stay late, put in extra work or do whatever is more than required.

Well, I mean, yeah. If a client had a negative review of their rep, I would want an explanation too.

That just seems like managing his subordinate's workflow. So he/she knows when things are supposed to be happening.
Bottom line: It doesn't sound like you're being micromanaged. It just sounds like you're being managed.

Another thought here ...
If employees are happy, they will take pride in their work and be owners of their work. And they'll stand side-by-side with their leader / company and they'll fight during the tough times and celebrate during the good times.
Taking care of your employees first means they will then put the customer, i.e. the company's "asset" first. But if an idiot manager oversteps his/her boundaries, they'll soon realize they'll be no one there to help them. And business will suffer.

Not at current job. I sometimes go a week or two without even seeing or talking to him.
Job before this was a different story. Managing partner was an extreme micromanager and ultimately why I decided to leave the firm. I am a grown arse man, I do not need someone monitoring my every move

My previous position had a boss that was somewhat of a micromanager. Now I work for someone who is really laid back and its a refreshing change.

I've met my boss twice. As long as things operate smoothly, reports are turned in on time, and his money keeps flowing steady, I never hear a word from him.
When his money flows, so does mine, and things remain laid back. Plenty of incentive to ensure things are run well.

Yeah, it just sounds like he is trying to gather information that he can use to make decisions. Sounds kind of efficient actually. Only a couple of the things OP listed sound like they might be a problem, but it depends on context.

It's hard to verbalize the feeling this engenders. It's not micromanagement in and of itself but it feels that way based on how it's handled. If some asshat gives you all 1s or 2s because they woke up on the wrong side of the bed or because of something completely beyond your control, it's thrown back in your face because you didn't a) inform him the customer was angry [not always apparent at the outset] or b) you failed in defusing the situation.
