You do understand the article references the updated guidelines from the American Academy of Otolaryngology -- Head and Neck Surgery Foundation, which is world's largest organization representing specialists who treat the ear, nose, throat, and related structures of the head and neck, right?

Again, it's difficult to find something more comical than a couple of people whose formal wear is a fishing shirt and wranglers argue with the foremost experts in the world.

I had an ear infection so bad that barely touching my ear lobe hurt bad. I went to the ENT. He told me to make a 50/50 solution of rubbing alcohol and white vinegar. I did that 3 or 4 times a day and it healed up a day or 2 later.

50/50 Mixture of vinegar(acetic acid) and hydrogen peroxide couple of times a day works pretty well for a mild swimmers ear (otitis externa). If it persists then you'll need, antibitoic drops +/- wick placement if it's bad.
Adults don't really get otitis media(that's what kids get), but if you do, you'd either oral antibiotics or an ear tube placed.

Adults get sinus infections, which can lead to ear infection (ear ache/ears feeling clogged) because of all the drainage. A simple trip to your doctor or urgent care, get a Z pack and/or a steroid shot (your choice). Gone in a few days.
Some of you seem to overcomplicate the frick out of something so simple. Costs you a copay and whatever the insurance doesn't cover on the shot if you decide to get it.

Is a misnomer. Adults can certainly get fluid in the middle ear space after having a URI. But it's non-purulent/non-infective and doesn't need abx to to treat. Will either get better with doing nothing or can be fixed quick with an aspiration/tube.
Again rarely does an adult get a true purulent otitis media.

Ok, so an upper respiratory infection. Not an "ear infection", per se. But there's still infection in your sinuses, which can certainly lead to ear pain. My ENT told me so.
The ABX don't treat the ear, they treat the infection in the sinus cavities, which helps with the ear pain.

I have to deal with that issue with my wife. She'll act like the world is ending and complain for a week, take a day or two off from work, etc. And then she goes to the doctor and what do ya know? Starts getting better within a few days.

Inner ear or outer ear? Totally different
Outer ear infections like swimmers ear, isnt usually bacterial. Inner ear infections are usually bacterial related in stuffy noise and sinus infections which swell passages. These require an antibiotic. Swimmers ear requires anti fungal.

The ENT I visited earlier this week said he preferred patients use a home remedy of half vinegar and half rubbing alcohol. Three times a day.

This.
My "ear beer" recipe is equal parts isopropyl alcohol, vingegar, and hydrogen peroxide.
I keep some mixed up in an old contact solution bottle and use it at first sign of something brewing in there (usual just tingling or itching.) Happens periodically after diving in questionable water...
