My wife washes the dishes prior to putting em in. So it is a non issue.
Sometimes I just wait till she isn’t home and unload it without running it.
Once in almost 20 yrs have I been busted

I load them in what ever direction I pick them up from out of the sink.

Handles up 100% of the time so no one gets poked when we're unloading it and putting the clean dishes away.

The silver wear basket in our machine is designed so the handles go down in to individual slots. So that's what we do. The basket attaches to the door, so when you open the door they are laying flat, no way to stab yourself.
If we had the more traditional style where the basket is in one of the main trays, I'd reverse it.

My dishwasher's cutlery holder only allows for forks and spoons up and knives (butter type, non sharp) down.
OT version: My Butler's service staff only hand wash and polish our silverware, daily.

This. Have so much more room on the bottom for dishes and pans. I can fit at least twice the stuff in my new dishwasher compared to the (at the time) very nice one from 1998 or so.

Knives are handles up.
Forks are handles down.
Spoons are a mix of both (if the spoons arent intentionally separated while loading, they will spoon together and not get clean)
We have the basket in the door. But the basket is removable. Im not sure why someone wouldnt walk the basket to the silverware draw, lay the basket on its side, and open the 4th wall on the basket so everything is accessible by the handle and flat.
Ive got 4 kids. We do a shite ton of dishes. Whatever is fastest and easiest is going to be the M.O. so long as they dont have to be washed a 2nd time.
