My dad was friends with a Vietnam vet who had two purple hearts. Got wounded by mortars on two separate occasions. He was pulling out tiny pieces of shrapnel up until he died a while back. They would work themselves out until on the edge of his skin where he could feel it then eventually would pull them out with tweezers like you would a splinter. He kept them in a little tabasco bottle.

Surgeons would just leave a bullet in you if it's not going to cause any additional damage to your body. It will usually become loculated with scar tissue.. Evidently, his was not very deep..
