the RRR relative risk reduction may be 25% but the ARR, actual risk reduction is 3-5%. which by big pharma standards is huge, but to Drs, just meh. For every 100 patients the dr treats with this, 3-5 will not have cardiac events.
What is the mortality data? would you take this knowing your chance of heart attack would go down, but you might not live as long?

Here's a start:
CDC - National Center for Health Statistics
For 2017, they give Rx drugs as 9.8%, but looking at the PDF tables, I'm not sure that includes drugs given as part of hospital care. I think I said 15% anyway, and I may have gotten that somewhere else. Anyway, the bullet points on that page are:
Download the PDF and look at Table 94. The annual growth rates have actually been moderating since 2000, reaching 45 year minima in 2015-2016.

It’s basically EPA from fish oil with an ethyl group added to it so it could be patented. The active metabolite of ethyl EPA is....plain old EPA.

The comment is really about statins, they may slightly reduce cardiac events, but don’t increase mortality. So while you may not die from a heart attack, you may die from one of the increased chances of diseases from statins, diabetes, dementia, cataracts…

If only it was so simple. My father was an ultra-marathoner up until the day he had a triple bypass at 62 years old. He ran every day and ate healthier than most people. Sometimes genetics fricks you. On the plus side, his running is what saved his life because he knew something wasn't feeling right his last month of running.

I eat sardines 4-5 times a week.
Don't need some high price prescription for them either.
