I think you can have a positive effect on anyone, which could lead to a chain reaction. Like if a person is having a really hard time and then their car won't start, they're probably pretty close to losing it. But if a kind stranger comes along and gives them a jump, it could turn their whole situation around. Maybe it restores their faith in humanity and they get their shite together. It can have a ripple effect if that person goes on to positively impact other peoples' lives, so in that sense yeah you can have an effect on the world.

Most people think you have to be famous to change the world but that's not true.

Stop trying to have an effect on the world; have an effect on people. Do enough of the latter and it will take care of the former.

No, none of us will. Truth be told, we're all just blinks in the annals of time. When we die, there may be some who grieve, but as time passes, we'll all be nothing but names on headstones.

30 years in EMS, birthed 11 children, intubated hundreds of pts, completely resuscitated close to 10-12 pts to being fully awake, and was there with the families during the worst of time,,,,,you be the judge.
