I'm no rocket scientist, but have done a lot with amateur rocketry in the past. My guess is they didn't throttle down (like they should have) before and during Max-Q.

Doubtful. More than likely just plain corruption and backstabbing that is the heart of most Chinese dealings. Faked inspections, falsified materials reports, sabotage due to internal politics...all the usual Chinese culture.
