Child in Car Seat Falls out of Moving Car as Mother Drives Away Unaware (Video)

I can't figure out how that happened. I understand they might be unable to properly secure the car seat, but the door opens too? I saw it this morning.

Mistake?
The driver slowed down at a green light after the door opened, then proceeded to drive off again.
Forgetting to buckle the seat in is a mistake. Driving off is not.
