There's no way I'd tell her to give up, but she needs to make some life style changes. I know some guys that are docs that are not that smart but they are dedicated to it, that is what it takes. It sounds like her dad's a doc and she is doing it because its all she's ever known but is not overly dedicated. Thus the Caribbean school and now failed tests.
If she has made it this far with this much money spent, its time to buckle down.
LOL at her hoping to have any particular field, its time for her to be happy taking anything she can get.

Step 1 was awful. It made the rest of school seem easy.
She does not need to throw in the towel just yet. Failing twice has shut doors for any prestige schools taking her.
At this point she needs to focus on passing the Step 1 exam. After that, she can find a "spot" in many medicine fields.
Lots of fields have residency programs with poor reputations, even surgery and ER medicine. She just needs to clear this hurdle and keep working her butt off in the future.
At the end, she can come out and be a clinician that enjoys her work.
She will never be in academics or research.
She will never be a thought leader or well recognized in her field.
But she was never going to do that from a Caribbean Medical school anyway.
This country still needs doctors in most every field. Tell her to keep working.
Step 1 is a barrier, not an indicator of future clinical skills.

Like I said, I think she's going to have to have a really good answer to the "why do you want to be a PA" question that she will inevitably be asked. She will have to explain her history to her interviewers in a way that won't have them rolling their eyes.
I'm sure some overpriced PA school would take her, but she might as well give the USLME another shot if she's willing to shell out med school prices to be a PA.

Physical Therapy deserves a look if she's genuinely interested in helping people get well; it's a well-regarded field, and won't be going away.

Yes. They also have more time to study for step 1, so their scores are viewed differently. Many Caribbean schools will cut out the losers to increase their match percentage so a lot of those failures don't reach the point where they interview for residency.
The pass rate is about 78% versus 95% for us/Canadian grads. The average step 1 of a matched us applicant was 230 in 2014 versus 225 for IMG. The average step 1 for unmatched IMGs was 213.

St Matthews has the highest Step pass rate. I know this because of the TD ad.

I really don't know. I know one other one that failed but she was further along and took it awhile back. Idk if she ever took it again and passed.

She will be ok. It will limit her residency spots and she can't go into the harder specialities, but it isn't a killer.

I don't think there is anything wrong with being honest and saying she should look at changing her future plans.
Not everyone is meant to be a doctor. The worst thing she can do is spend a great deal more time and money chasing a dream for nothing. I know she is devastated, frustrated, embarrassed, and dejected right now. However, years down the road she will be very happy she moved on to something else.
