The black areas on CT scans/X-rays are actually gas filled areas (usually air in the case of lungs). Think about it as air is soooo thin as to not impede/interact with radiation. Very very few particles, per square inch let's say, than compared to muscle or fat or bone. Less density = darker; more density = brighter (lungs black, bones white).
So, to answer more precisely about smoking damage.....scarred tissue or tumorous growths would be more dense than normal air filled lung tissues and would show up as lighter areas instead of black.
ETA: Those black bubbles in the intestines on the left fatass, er, patient, means you may wanna open a window soon, she gonna gas fill the area soon.
