There is no mention how many comorbidities a patient may have that also have lead to their death. Relatively healthy patients do not die from UTIs and are unlikely to get surgical site infections. Someone else will take care of you and if you do poorly it will be their fault.

Edit to add that leapfrog website you linked has the hospitals with the worst reputations in my area scoring A's and the two better hospitals scoring C's. I work at all of them so I could care less which hospital gets what score but it just proves to me that those scores mean.
