Any LEO here fill out crash reports? Update got answers offline

Here's the first couple...
1- Is text input character limited as it suggests by the boxes in the main form? IE, if there are 15 boxes for name, does Alexander The Great get entered as "Alexander_The_G"?
2- What is the icon-area for vehicle configuration section? How is it used in the form? IE, if a passenger car (A), motorcycle (E) and school bus (I) get in a wreck, are they circled in this section? Are they referenced later in the form by letter?
3- Is Agency on bottom of page 1 same as agency at top right of page 1?
These types of questions... something someone who has filled out the forms many times would know how different sections are used.

I read a bunch of accident reports.
I would think that text input in these boxes are limited by the boxes. If more information is needed, it can be included in a narrative.
I don't have any idea what this is unless you are trying to draw a diagram. I do not know which section you are referencing.
I do not see a space for "agency" at top right of page 1.

1. No
2. Every vehicle involved in the crash will have a separate vehicle page where the configuration and other general info about the car is noted. So say vehicle 1 is a Ford fiesta.
A for passenger car, x for no trailer or cargo, 4 doors 2 axles 4 tires.

THIS. But I still think more context is needed. Because if we knew the end result your trying to accomplish we can answer the questions accordingly. Those of us who know a bit about the LA Crash report system. But I can tell you that most if not all Officers fill out crash reports on a computer system these days. Very rarely will you still do a hand written crash report. Due to drawing out how the crash happened and narratives in it. Computers make it easier.
