Michael Myers is pretty much pure evil if that's what you're talking about. I'd also put John Doe up there.

Oddly enough, much like Margaret Hamilton (Wicked Witch) Ratched's actor, Louise Fletcher, is widely regarded as one of the nicest people in Hollywood. A lot of great villains seem to be played by the nicest people around.
Tom Felton who was a perfect snively little arse weasel as Draco in the Harry Potter movies is supposed to be just wonderful to work with and Tom Hiddleston (the villain of choice for Marvel as Loki) is renowned for how happy he is to meet fans.

The Devil's Advocate. Not the greatest movie ever, but not a bad watch. I actually think Pacino/the writers went more for a Miltonian Satan, who is more nuanced than pure evil. He plays the part of the rejected Satan with a grudge rather than just evil for evil's sake.

She did a wonderful job as a villain on Deep Space 9 as well. Her character just oozed "You poor fool I know what is best for you".
